In consideration for our other patients, there is a $90 Fee for the following:
- Late Arrivals, you will be asked to reschedule
- Cancel less than 24-Hours in advance
- No-Show
At Rocky Mountain Ear Center every member of our team is a professional to ensure you receive the best possible quality care. We take pride in our ability to serve you and look forward to a mutually satisfactory relationship.
Rocky Mountain Ear Center accepts new patients who are being referred by another medical doctor, audiologist or specialist. We do accept Medicare and Medicaid patients on a limited basis. We ask that all previous medical records, audiograms, and radiology scans be forwarded to our office for review prior to your appointment.
Please have a copy of your current insurance card(s) and a photo ID with you. You must review the current health insurance information and patient intake information each time you are seen. Information changes frequently and it is necessary for us to have you complete all of the requested paperwork in its entirety at each visit.
Email Policy
We welcome our patients to communicate with any member of the Rocky Mountain Ear Center and/or Hearing Aid Center staff via email. When using email as a form of communication it is essential to note that email is not to be used for urgent problems or if a timely response is required. When using email as a form of communication it is important to know that emails may be opened by any member of our staff, and that emails may require three (3) business days for a response. Prescription requests and refills will not be provided through the email format. If you would like to communicate via email you must complete the following form:

Care of Minors
Patients who are less than 18 years of age will require consent for treatment from a parent or other legal guardian. If the patient is to be accompanied to their appointment by an adult other than a parent or legal guardian, the parent or legal guardian may write a note and complete the Parental Consent For Medical Care form stating who will accompany the minor patient to the appointment. Please fax both the note and the form to our clinic at (303) 806-6292 prior to the patient being seen.
Payments
All Co-pays, payment for supplies and some audio and medical services are due at the time of service. Please have a copy of your current insurance eligibility cards and a photo ID with you. We accept cash, checks, Visa and MasterCard. We do not accept self pay patients without prior authorization.

Out of Town Patients
If you are being referred to our clinic from outside the Denver Metro area our policy requires that a patient history and other information be obtained from you before scheduling your appointment. Our staff will take this information and request that your records be sent to us ASAP. Once we have received your records, our physicians will review the information and determine if any additional testing needs to be done prior to your appointment. This policy eliminates the process of the patient having to make multiple trips back and forth for additional testing. We may not be able to schedule your appointment at the time of your call, but someone will call you back to schedule the appropriate appointments after the records have been reviewed. We appreciate your understanding and patience during this process.
Automobile Accidents and Workers’ Compensation
We do not accept patients with claims for automobile accidents or Workers’ compensation.
