Patients must have a hearing/balance/ear-related diagnosis and referral from an outside practitioner to see one of our physicians or audiologists. Please call to schedule an appointment with an audiologist.

For medical records requests, please call the main number (303) 783-9220 and select option 2 or fax request to 303-806-6292. Routine requests for medical records are processed within one to two weeks.
